Working Together
Thorough Occupational Therapy (Thorough OT) is a Tasmanian home visiting service covering Hobart and surrounding areas.
Thorough OT service CHSP, HCP and NDIS adult and elderly people experiencing a change in their abilities to do the practical everyday activities of daily living, from getting out of bed to getting off the toilet.
Our principal clinician Julia has over 20 years’ experience in this area.
As a guest in the person’s home and in considering their unique circumstances, at Thorough Occupational Therapy we value working with individuals to review their presenting difficulties in their own environment.
Our overall aim is to review options to enable a person to remain in their home as independently and safely as possible.
Any change in function, be it due to frailty, aging or a specific disability can be harrowing for an individual and those close to them. Often people feel a loss of choice and control over their lives and actions. A person’s pride can be dramatically affected by a change in their abilities. It is often a combination of the loss of pride choice and control that affect a person’s ability to consider and try options to increase their independence and safety. Sometimes people feel embarrassed and frustrated that they can’t do what they used to be able to do.
At Thorough Occupational Therapy we undertake a person-centered approach engaging the individual in evaluating the risks and considering the outcomes to keep them safe living at home.
Home Visit and Service Process
Referral
The referral received will determine what type of assessment is undertaken:

We will arrange a home visit with the person referred to or their relative or support person.
Home Visit and Assessment
We will travel to the persons home and undertake a home safety assessment and review activities with the person through listening to them describe how they do something and through practical observation watch the person undertake the activities of daily living. From getting in and out of the front door to getting out of the shower.
Following this we review options with the person that may reduce risk and increase safety and support for them with the aim of keeping people at home.
Report and Recommendations
As a service we will complete an Occupational Therapy Assessment Report containing a summary of risks identified and our recommendations.
In acknowledging choice and control we will detail whether the person wishes to consider the recommendation or not at the time of the report.
As a person-centered occupational Therapy practice, we consider that it is the person’s home and that they continue to live with the dignity or risk and decide what recommendations that they feel are right for them and that they wish to consider.
Follow Up
Following the report submission, we may be involved in further work with the individual for actions such as set up and orientation to safe use of equipment (assistive technology), review of identified risks such as pressure injury or moving and handling, and if major or minor modifications have been recommended.
